provides on-site rehab services that include OT, PT and Speech therapies. Our team of seven highly skilled therapists provides six days a week with a 7-day schedule when required. Treatment plans consider each resident's unique needs whether short-term, long term or intensive therapy is recommended.

The short-term person who goes home

      • Short-term therapy covers six days of therapy a week as needed, allowing the resident to return home as soon as possible.

Residents who need long term care

      • Long-term therapy focuses on a smooth transition into the long term care setting.

Receiving therapy three to five days a week a resident is able to reach their highest level of independence.

The requirements of individuals who need an extended period to rehab at a slower pace

      • Notre Dame has a high success rate with the medically complex resident, who receives therapy six-days a week and typically stays longer than 30 days, but eventually returns home or to assisted living.

For residents returning home the treatment plan includes a specialized home visit in which an assessment of the environment helps formulate a specific plan designed to ensure safety and success for the resident's return.
